AI 100-1 -- Function-Level Risk Assessment

Organization Assessed: Athena Logistics Inc.

AI System: SupplyGuard v4.1 (Supply Chain Anomaly Detection)

Risk Profile: High impact -- critical infrastructure supply chain decisions

Assessment Body: Pacific Risk Advisory LLC

Assessment ID: PRA-2026-0312

Assessment Standard: NIST AI RMF (AI 100-1, January 2023)

Report Date: 2026-10-06

Assessment Window: 2026-07-15 through 2026-10-01 (78 days)

1. Executive Summary

Scope of Assessment

This assessment evaluates Athena Logistics' implementation of the NIST AI Risk Management Framework across all four core functions (GOVERN, MAP, MEASURE, MANAGE) for their SupplyGuard v4.1 system. SupplyGuard is an anomaly detection engine used to identify supply chain disruptions across 14 distribution centers serving federal logistics contracts.

Evidence Sources

  1. Cryptographic witness evidence (SWT3): 20 SWT3 witness anchors covering 16 procedures, assessed via the audit portal over a 78-day assessment window. Each anchor was independently verified using SHA-256 fingerprint recomputation.
  2. Document review: AI governance charter (v3.0, dated 2026-05-01), risk register, model validation reports, incident response plan, and AI system inventory.
  3. Interviews: 3 sessions with the Chief Risk Officer, AI Engineering Lead, and Head of Supply Chain Operations. Interview notes on file (ref: PRA-INT-0312-01 through 03).

Overall Risk Posture

MANAGED WITH IDENTIFIED GAPS. 3 of 4 RMF functions are substantially addressed. One gap identified in MANAGE (incident reporting not yet operational). One area for improvement in MEASURE (adversarial testing scope).

2. Assessment Methodology

Assessment Framework

This assessment was conducted per PRA internal procedure PRA-QMS-P-008 "AI Risk Management Assessment Using Cryptographic Evidence" (Rev 1.0, effective 2026-06-01), aligned with NIST AI RMF (AI 100-1) and NIST AI RMF Playbook guidance.

SWT3 Evidence Verification Method

SWT3 witness anchors were verified using a two-step process:

  1. Portal verification: All 20 anchors were checked via the public verifier at sovereign.tenova.io/verify. Each returned VALID.
  2. Independent recomputation (5 anchors): Five anchors (one per RMF function plus one cross-cutting) were selected for manual SHA-256 fingerprint recomputation using OpenSSL 3.x. All five matched.

Interoperability Verification

The SWT3 fingerprint formula is implemented in 7 independent SDK implementations and verified against a shared test vector suite. Independent recomputation used OpenSSL 3.x, confirming vendor-independent reproducibility.

Temporal Scope

The assessment window was defined as 2026-07-15 through 2026-10-01 (78 days). Of the 16 procedures assessed, 14 had continuous anchor coverage. Two procedures (AI-REDTEAM.1 and AI-INCIDENT.1) had absent coverage, documented in the MEASURE and MANAGE findings below.

Evidence Sufficiency Determination

SWT3 anchors were accepted as sufficient evidence for automated monitoring and operational control objectives. Governance structure, organizational culture, and stakeholder engagement objectives were assessed through document review and interviews. Where both sources covered the same objective, both were considered.

3. Function-Level Findings

GOVERN -- AI Risk Governance

Subcategories assessed: GOVERN 1.1, 1.2, 1.3, 1.5, 2.1

ADDRESSED Athena Logistics maintains a documented AI governance framework with clear accountability assignments, regular review cadence, and defined escalation paths. The AI governance charter (v3.0) establishes roles, responsibilities, and risk appetite for AI system deployment.

MAP -- AI Risk Identification and Context

Subcategories assessed: MAP 1.1, 2.1, 2.3, 3.5

ADDRESSED Risk identification and categorization processes are established. The organization maintains a risk register covering technical, ethical, and operational AI risks. Bias detection is operational for demographic fairness in supplier selection algorithms. Impact assessments are conducted for all high-risk deployments.

MEASURE -- AI Risk Measurement and Monitoring

Subcategories assessed: MEASURE 2.5, 2.6, 3.1

SUBSTANTIALLY ADDRESSED Performance monitoring and drift detection are operational and continuously witnessed over the assessment window. Guardrail enforcement is active with 4 guardrails deployed.
Area for Improvement (IMP-001) Adversarial testing (AI-REDTEAM.1) has not been conducted during the assessment window. The organization reports annual red team exercises but the most recent campaign predates the assessment window. Suggested action: increase adversarial testing frequency to semi-annual, particularly given SupplyGuard's critical infrastructure role.

Assessor notes: The continuous SWT3 evidence for AI-DRIFT.1 and AI-PERF.1 across 78 days provides strong assurance of ongoing monitoring (MEASURE 2.5). The absence of AI-REDTEAM.1 within the window does not indicate non-compliance but limits the assessor's ability to evaluate MEASURE 2.6 (adversarial robustness) with current evidence.

MANAGE -- AI Risk Response and Recovery

Subcategories assessed: MANAGE 1.3, 2.1, 3.1, 3.2, 4.1

PARTIALLY ADDRESSED Human oversight mechanisms are operational. Lifecycle management is documented and witnessed. Override capabilities are tested and functional.
Gap Identified (GAP-001) AI incident reporting (AI-INCIDENT.1) is not operational. The organization has drafted an AI incident response plan but has not yet implemented the reporting mechanism. MANAGE 4.1 requires a defined process for responding to and recovering from AI incidents. The incident response plan exists as a document but no incidents have been reported or tested through the system during the assessment window.

Supplementary evidence: Interview with Head of Supply Chain Operations (PRA-INT-0312-03) confirmed human override procedures are exercised monthly. Incident response plan (draft v0.9) reviewed but found incomplete -- no escalation thresholds, no notification timelines, no post-incident review process.

Suggested action: Implement AI incident reporting mechanism per MANAGE 4.1. Complete incident response plan with escalation thresholds, notification timelines, and post-incident review procedures. Conduct at least one tabletop exercise. Target: 60 days (2026-12-05).

6. Conclusion and Recommendation

Risk Posture: MANAGED WITH IDENTIFIED GAPS

SupplyGuard v4.1 demonstrates substantial alignment with the NIST AI Risk Management Framework across GOVERN, MAP, and MEASURE functions. One gap (GAP-001) was identified in the MANAGE function regarding incident reporting. One area for improvement was noted regarding adversarial testing frequency.

The assessment body recommends:

  1. Address GAP-001 within 60 days (target: 2026-12-05)
  2. Submit completed incident response plan and tabletop exercise results for review
  3. Follow-up assessment of MANAGE 4.1 (January 2027)

The use of SWT3 cryptographic witness evidence provided continuous assurance over the 78-day window for 14 of 16 procedures assessed, offering stronger temporal coverage than periodic manual evaluations.
